Healthspan Neurology, PC is a fee for service practice. No insurance is billed. Dr. Gajaweera provides in-office and in-home visits in Rochester, NY. Visits are in-person, directly with Dr. Gajaweera. Telehealth consultations with Dr. Gajaweera are also available for patients within NY state. No referral is needed but patients must have a primary care doctor. Appointment fees vary based on the type and length of visit. Payment is required at time of booking.

The Dementia Preventive Program is ideally implemented as a 6 month comprehensive program to strategically analyze, stratify and mitigate one’s personal dementia risk factors. This process takes time and is best done over multiple visits.
6 Month/6 Visit Program - $2,200 one-time fee
6 visits with Dr. Gajaweera to be completed within a six month time period (~8 hours total)
3 × 1.5 hour visits in-person or video visits
3 × 30” phone visits
Home Visit Option
Available within a 10-mile radius — add $100 per visit to above.

To provide each patient with the time and attention necessary Healthspan Neurology, PC and Dr. Gajaweera do not participate in any commercial, Medicare, Medicaid or Tricare insurance products. Healthspan Neurology is not a concierge practice. There are no premiums or monthly fees beyond what is listed above for a specific service.
HSA and FSA payments may be accepted for some visits. Be aware that in general, insurance will not cover preventive neurologic care.
Most lab testing and imaging WILL be covered by your insurance and Dr. Gajaweera will always aim to obtain insurance authorization from your insurance company or inform you of anticipated 3rd party charges.
If you have a commercial insurance product (NOT Medicare, Medicaid or Tricare) at the conclusion of your appointment, you may ask to be provided with a “superbill” that you may submit to your insurance company for possible partial reimbursement. Healthspan Neurology, PC and Dr. Gajaweera can in no way guarantee insurance reimbursement.
Cancellation Policy
We understand plans can change.
If you need to reschedule your appointment, you must do so prior to two business days in advance of your visit. You will then have three months to use the appointment credit.
If you cancel your appointment with no intention of rescheduling or do not reschedule within the initial three month period, you will be offered a 50% refund.
Late cancellations (within two business days of the appointment) will not be refunded or credited.
Healthspan Neurology, PC may review and adjust its fee schedule periodically to account for changes in operational costs and market conditions.
