Pricing & Policies
Many patients tell me they leave a single visit having learned things that never came up in years of other doctors' appointments. Insurance reimburses a 20-minute visit focused on one problem. That's not how I practice medicine. Your neurologic symptoms don't exist in isolation of your whole body health, and neither does your brain health.
Effective April 1, 2026
All visits are with Dr. Gajaweera directly — in person, via telemedicine (NY State), or at home. Payment is due at the time of booking. Across all services, the effective rate averages around $300–$350 per hour.
Home visits available within 10 miles of the Rochester office — add $100 per visit to any service.

Insurance & reimbursement. Healthspan Neurology does not bill any insurance plan, including Medicare, Medicaid, or Tricare. There are no monthly membership fees. HSA and FSA funds are accepted. If you have commercial insurance, a superbill can be provided at the conclusion of your visit for possible out-of-network reimbursement — though reimbursement cannot be guaranteed.
Lab testing & imaging. Most lab work and imaging ordered during your care is billed directly to your insurance. Dr. Gajaweera will make reasonable efforts to obtain prior authorization and will inform you of anticipated costs before testing is ordered. Up to 30 minutes of administrative time per test is absorbed by the practice; some insurers require additional follow-up beyond what can be accommodated.
Cancellation policy. Please reschedule at least two business days before your appointment — your payment will be held as a credit valid for three months. Cancellations without plans to reschedule, or unused credits after three months, receive a 50% refund. Cancellations within two business days of your appointment cannot be refunded or credited.
Fee schedule. Fees are reviewed periodically to account for changes in operational costs. Current pricing is always posted on this page. By receiving services at Healthspan Neurology, PC, you acknowledge that fees are subject to change.
