Healthspan Neurology is a direct-pay, fee-for-service practice. No insurance is billed for visits. All appointments are with Dr. Gajaweera directly — in person at our Rochester office, in your home, or via telemedicine for patients anywhere in New York State. No referral is required, though all patients are asked to maintain a relationship with a primary care provider to support continuity of care.
Payment is required at the time of booking.
Services & Fees
Dementia Preventive Program
A six-month, four-visit program designed to assess, prioritize, and address your personal dementia and Alzheimer's risk factors.
Program fee: $2,700 A three-payment option is available: $950 at enrollment, $950 at your second visit, and $950 at your third visit.
Includes four extended visits with Dr. Gajaweera over six months — in person or via telemedicine. The first visit is typically 90 to 120 minutes. Visits two through four are booked as 90 minute blocks; actual visit length varies based on what's needed.
Follow-up visit: $650 — $550 when scheduled before program completion. 75 to 90-minute in-office or telemedicine visit.
Home visit option: add $100 per visit Available within a 10-mile radius of the office.
Mind Over Menopause™
Initial visit:
$570 90-minute in-office or telemedicine visit. Two hours are reserved to ensure adequate time.
Follow-up visits:
$450 60 to 75-minute in-office or telemedicine visit. 90 minutes are reserved to ensure adequate time.
Home Visit Option:
add $100 per visit Available within a 10-mile radius of the office.
Neurologic Consultation
Initial visit:
$570 90-minute in-office or telemedicine visit. Two hours are reserved to ensure adequate time.
Follow-up visits:
$450 60 to 75-minute in-office or telemedicine visit. 90 minutes are reserved to ensure adequate time.
Home Visit Option:

Insurance & Reimbursement
Healthspan Neurology does not participate in any commercial, Medicare, Medicaid, or Tricare insurance plans. This is not a concierge practice — there are no monthly premiums or membership fees beyond the visit fees listed above.
HSA & FSA: Many visits are eligible for payment using HSA or FSA funds.
Lab testing & imaging: Most laboratory testing and imaging ordered during your care will be billed directly to your insurance. Dr. Gajaweera will make reasonable efforts to obtain prior authorization and will inform you of anticipated out-of-pocket costs before testing is ordered. Please be aware that insurance authorization can be a time-intensive process. We allocate up to 30 minutes of administrative time per test toward this effort — in some cases, insurers require additional follow-up that falls outside what we are able to absorb.
Out-of-network reimbursement: If you have a commercial insurance plan (not Medicare, Medicaid, or Tricare), you may request a superbill at the conclusion of your visit to submit to your insurer for possible partial reimbursement. Healthspan Neurology cannot guarantee reimbursement.
Cancellation Policy
We reserve extended time for every patient, and late cancellations affect our ability to serve others.
If you need to reschedule, please do so at least two business days before your appointment. Your payment will be held as a credit, valid for three months.
If you cancel without plans to reschedule, or do not use your credit within three months, a 50% refund will be issued.
Cancellations made within two business days of your appointment cannot be refunded or credited.
Fee Schedule
Healthspan Neurology reviews its fee schedule periodically to account for changes in operational costs. By receiving services at Healthspan Neurology, PC, you acknowledge that fees are subject to change. Current pricing will always be posted on this page.
