Comprehensive Cognitive Evaluation: Memory & Cognitive Symptom Diagnosis

If you or someone you love is noticing memory lapses or other cognitive changes, Comprehensive Cognitive Evaluation offers a focused, two-visit path to answers — a thorough diagnostic workup, a clear explanation of what's happening, and a plan for what comes next. This evaluation is available in person only, at our Rochester, NY office.

Dr. G at Healthspan Neurology was the first doctor to get the answers I was looking for after others had missed it — I can’t overstate how life-changing that was for me. She responds quickly, listens thoughtfully, and always makes me feel supported. Her team-based approach and trusted referrals have given me peace of mind and better results than I ever expected. Most importantly, Dr. G thinks outside the box. She’s mission-driven and genuinely empowers me to take control of my care in a way that fits me. Who This Is For

Comprehensive Cognitive Evaluation is designed for anyone noticing real, ongoing cognitive symptoms now — not risk factors or family history alone, which is what the Cognitive Longevity Program addresses. You may benefit if you or a loved one:

  • Are experiencing new or worsening memory lapses, confusion, or word-finding difficulty

  • Have been told by family or friends that something seems different

  • Want a clear diagnosis rather than continued uncertainty

  • Need a plan for next steps — whether that's ongoing management, referral, or reassurance

How It Works

Two visits, both in person at our Rochester, NY office.

Visit 1 — Comprehensive Assessment
Scheduled at intake, this 90 to 120 minute visit includes a full history, a complete neurologic exam, and traditional (non-digital) cognitive testing to establish a clear picture of what's happening.

Visit 2 — Results & Recommendations
Once your results are ready, we schedule a follow-up visit — up to 90 minutes — to review your findings together, explain the diagnosis, and lay out recommendations for ongoing care, including referral to specialists or continued management as appropriate.

How This Is Different

Standard neurology appointments are often brief, focused on a single symptom, and disconnected from the broader picture of a patient's health. That model works for straightforward cases. It frequently falls short when the concern is memory or cognition.

At Healthspan Neurology, evaluations are comprehensive and unhurried. Appointments allow time to review prior records, listen carefully, and understand the full context of your life — not just the symptoms that brought you in. Addressing cognitive concerns means understanding what matters to you: your priorities, your goals, and the environment you live in. The brain doesn't function in isolation — cognitive symptoms are frequently influenced by medications, sleep, hormones, metabolic health, and mood, and all of it is part of the conversation.

With more than 25 years of experience in the Rochester medical community, Dr. Gajaweera also maintains an extensive network of trusted specialists. When referrals or coordinated care are needed, they are made thoughtfully — to people she knows and trusts.
